The first thing that comes to mind is the lighting situation. Is the tegu receiving sufficient lighting: UVA / UVB, with the temperatures and humidity at consistent levels?

Our Zeus went into hibernation in January ( we got him in December ).He looked very healthy when he went into hibernation.I know that Tegus suppose to come out of hibernation in March.I got very worried when it was the end of march and he was still sleeping. So I pm Rick ( the Admin from this forum and he is the Breeder of some very tame Tegus ) and asked him what I should do.He sad it was ok for me to dig him up.So I did.He was very skinny. He wouldn't eat any thing either.I also was worried that when he is basking but didn't eat any thing that he would get dehydrated so I force fed him some chicken and turkey/gravy baby food with some calcium and vitamine powder. Two days ago he ate a pinky and yesterday he ate a fuzzie. He looks a lot better now.
