Possibly impaction (swallowing of substrate)

What substrate are you using. Young Leo's have a habit of ingesting lots of substrate as they are not overly brilliant at catching food.

Other than that, may be worth taking one to a vet who could maybe help you diagnose the cause of death.

Over here we have young leo's on paper towles so they can't eat the substrate together with their food.
Maybe you want to try that instead of the normal substrate you use for adults?
